Annex XII transparency package

Document the information your downstream AI system providers need to integrate your model and comply with their obligations.

This form collects the information required under Annex XII of the EU AI Act for disclosure to downstream AI system providers. Fields marked with AXII reference codes correspond directly to Annex XII sections. Annex XII is distinct from the internal technical file required under Annex XI — it governs what the GPAI model provider must make available to providers who build AI systems on top of the model. Provectio will measure your documentation against the applicable requirements and return a governed receipt with findings and integrity hashes.

02 Model General Information Annex XII §1(a)–(h) ▲

Complete this field where specific software versions govern correct or supported use of the model. Leave blank if not applicable.

For MoE models, state total and active parameters separately in the field below.

The Article 53(2) open-source exemption from certain Annex XII obligations does not apply to models with systemic risk. Provectio will measure applicability from the documentation provided.

03 Integration Information Annex XII §2(a)–(c) ▼

URL to the provider's integration or API reference documentation, if available.

Provide the level of detail needed by downstream providers to understand the data basis of the model. This field is not required where the open-source exemption applies and the model does not pose systemic risk.

04 Document Upload ▼

Accepted formats: PDF, Markdown, plain text, DOCX. Submit the downstream transparency document, model card, or equivalent disclosure document. This will be measured against Annex XII requirements.

Optional. Submit API reference documentation, OpenAPI specification, or integration guide.

Optional. Further documentation relevant to Annex XII coverage — for example, acceptable use policy, licence text, or data sheet. Multiple files accepted.
